This paper has been written to stimulate a debate in our specialty. A debate that we feel is long overdue. We are greatly concerned that recruitment into our specialty is in terminal decline. Recently only 30% of posts were appointed to at ST3 national selection. And it is not a blip. We would welcome a lively debate through the pages of the BJOMS and perhaps a dedicated day at the RCS hosted by BAOMS.

Stats and ladders: Injury risk and outcomes following falls from ladders
Fatal lower level falls commonly result from ladder fall injuries (LFIs), an often-avoidable injury. We hypothesized that older patients’ injury severity differs from younger patients falling from the same mechanism with fall height determining overall morbidity. A retrospective review was completed of all traumatic LFIs during a 6-year period resulting in 178 patients. Nucleophosmin (NPM1) has been suggested to be involved in the pathophysiologic mechanism of inflammatory disorders. We measured the expression level of NPM1 in nasal polyp (NP) tissues of patients with chronic rhinosinusitis with nasal polyposis (CRSwNP).
