Small lessons during COVID having a large impact on surgical management
In the past, in the Oral and Maxillofacial (OMFS) department in Ninewells hospital Dundee, we have placed a tracheostomy in patients undergoing free flap surgery as the standard protocol. During the COVID pandemic this has changed. The patients now receive an extended intubation period the night of their surgery. Could this be a positive and a permanent change?

Efficacy of pre-expanded forehead flap based on the superficial temporal artery in correction of cicatricial ectropion of the lower eyelid
Lower eyelid retraction or ectropion bring suffering to the patients both physically and mentally. Efforts to explore pertinent surgical treatments are still needed. Here we have described our preferred surgical technique for lower eyelid ectropion repair with pre-expanded island forehead flaps or pedicled forehead flaps. Re: Evidence regarding corticosteroid use in deep cervicofacial infections of odontogenic origin
We thank Chavez-Rimache et al. for their letter, in which they highlighted the importance of further study into acute steroid administration to patients with odontogenic cervicofacial infections. As they highlighted, existing systematic reviews and meta-analysis of other head and neck conditions suggest benefits with few side effects.

Evidence regarding use of corticosteroids in deep cervicofacial infections of odontogenic origin
The use of corticosteroids as a combined therapy with antibiotics in deep cervicofacial infections of odontogenic origin is controversial. On one hand, it is considered that parenteral administration of high doses of corticosteroids, once or twice, can improve recovery in trismus, reducing pain as a result of its anti-inflammatory effect. On the other hand, there is concern regarding its adverse effects, such as immunosuppression, which could also aggravate the infection by weakening the immune response.
