Abstract Objectives The aim was to evaluate the overall occurrence and potential clustering of biological and technical complications in implant dentistry. Material & Methods 596 patients provided with implant‐supported reconstructions were evaluated for the occurrence of (i) technical complications, (ii) peri‐implantitis and (iii) implant loss during a period of 9 years. Time and type of event were scored, and potential risk factors were explored through parametric modelling of survival...

Gingivitis is a highly prevalent oral condition that can be studied in humans via the 21‐d experimental gingivitis model, which allows for investigations into the induction and resolution of gingivitis. In this study, we used the autolysis of saliva as a source of peptides to predict the activity of human proteases in saliva during induction and resolution of inflammation. Healthy volunteers, with no remarkable oral or systemic conditions, were recruited into the study and stimulated saliva samples...
In the dental pulp surrounded by rigid dentinal walls, an increase in fluid volume will be followed by a rapid increase in interstitial fluid pressure. To maintain pressure homeostasis, a fluid drainage system is required. The dental pulp and apical periodontal ligament lack lymphatic vessels, and the questions are how the transport can take place inside the pulp and where the lymphatic vessels draining fluid from the apical periodontal ligament are located. The drainage of fluid within the pulp...

The clinicopathological aspects and prognostic factors of 40 patients with adenoid cystic carcinoma (ACC) of the intraoral minor salivary glands, treated between 2007 and 2017 at a single institution, were evaluated retrospectively. Twenty-six were female and 14 were male, and they ranged in age from 26 to 81 years (median 55 years). ACC occurred mainly in the palate, with 54.8% of cases presenting T3–T4 lesions. Curative surgery was performed in all patients, and 62.5% of patients were treated with...
Medication-related osteonecrosis of the jaw (MRONJ) is a complex disease which can be associated with multiple morbidities and is challenging to treat. This review evaluates the literature on the role and efficacy of teriparatide (TPTD) as a treatment for MRONJ. The clinical, radiological, histopathological and serological parameters used to assess treatment response have been described. Electronic databases were searched to retrieve articles (April 2005 and April 2020) based on strict inclusion...

Sabbattini first described a cross-lip, full thickness transfer of tissue in 1838 and a similar flap design was later popularised by Abbe in 1898.1,2 Since then, it has been widely adopted and modified for the reconstruction of upper or lower lip defects.
Cervicofacial infections of dental aetiology can be life-threatening and with the closure of dental practices following the onset of the COVID-19, it would be anticipated that their prevalence presenting to maxillofacial surgery would increase and services may be overwhelmed, with patients presenting later with a potential subsequent increase in morbidity. A retrospective analysis of patients with cervicofacial infection of dental aetiology referred to maxillofacial surgery during the initial six...
In the past, in the Oral and Maxillofacial (OMFS) department in Ninewells hospital Dundee, we have placed a tracheostomy in patients undergoing free flap surgery as the standard protocol. During the COVID pandemic this has changed. The patients now receive an extended intubation period the night of their surgery. Could this be a positive and a permanent change?
Osteoradionecrosis of the jaw (ORNJ) is one of the most devastating and progressive complications of head and neck radiotherapy. It can cause emaciation, deformity, and pathological fractures, resulting in decreased quality of life. The aim of this study was to evaluate the preoperative index and outcomes of treatment for ORNJ. A retrospective study of 252 ORNJ cases treated at one institution between January 2010 and January 2018 was made. The abnormal items from the preoperative examination and...
In 2018, NHS England released guidelines restricting the prescription of over-the-counter medicines (OTC). This was a project initially targeted at primary care, but we felt that it could also be used to improve services in secondary or tertiary care. We were interested to investigate our OTC prescribing pattern within a busy maxillofacial department and therefore carried out a service review. All outpatient prescriptions were reviewed between October 2017 and October 2018. The review was then repeated...
Achieving compression in the fracture line gap by open reduction and internal fixation leads to more primary bone healing and therefore hastens the healing process and patient's rehabilitation. We aimed to evaluate the application of compression by a modification in screw insertion in miniplates to improve the efficiency of the previous relevant methods. In this in vitro experimental study 20 sheep hemimandibles were prepared. Following intentional fracturing of the hemimandibular bodies, the specimens...
Lower eyelid retraction or ectropion bring suffering to the patients both physically and mentally. Efforts to explore pertinent surgical treatments are still needed. Here we have described our preferred surgical technique for lower eyelid ectropion repair with pre-expanded island forehead flaps or pedicled forehead flaps. Between July 2007 and March 2017, a retrospective review of 12 patients who suffered from cicatricial lower eyelid ectropion as a result of trauma, surgery, or infection was conducted.
We thank Chavez-Rimache et al. for their letter, in which they highlighted the importance of further study into acute steroid administration to patients with odontogenic cervicofacial infections. As they highlighted, existing systematic reviews and meta-analysis of other head and neck conditions suggest benefits with few side effects.
The 8th edition of the UICC TNM (UICC 8) staging rules for oropharyngeal squamous cell carcinoma (OPSCC) acknowledges dichotomous disease biology based on the human papillomavirus (HPV) tumour status. This retrospective study was undertaken to validate those staging rules in a single UK treatment centre. Given a recent resurgence of interest in primary surgery for OPSCC, a secondary objective was to identify subsets of patients who might benefit. Patients presenting with OPSCC between 2010 and 2017...
The use of corticosteroids as a combined therapy with antibiotics in deep cervicofacial infections of odontogenic origin is controversial. On one hand, it is considered that parenteral administration of high doses of corticosteroids, once or twice, can improve recovery in trismus, reducing pain as a result of its anti-inflammatory effect. On the other hand, there is concern regarding its adverse effects, such as immunosuppression, which could also aggravate the infection by weakening the immune response.
Synovial chondromatosis (SC) of the temporomandibular joint is a rare, benign, pathological condition in which cartilaginous nodules form in the synovial lining.2 The aetiology for the primary SC is unknown, but has been suggested to follow trauma, embryological disturbances, or infection.1,3 Symptoms include pain in the temporomandibular joint (TMJ), limited mouth opening, swelling, occlusion changes, and deviation.3 Computed tomography (CT) and magnetic resonance imaging (MRI) are leading diagnostic...
Cell sheets from bone marrow mesenchymal stem cells (BMSC) have been widely used in the field of bone tissue engineering, although their source remains a challenging issue. In this study, adipose-derived stem cells (ADSC) were induced to differentiate into osteoblasts, and the incorporation of coral scaffolds with ADSC sheets for bone formation at an ectopic site was also investigated. First, ADSC isolated from inguinal adipose tissue of New Zealand rabbits were cultured for two weeks without passaging...
The objective of the present systematic review was to test the hypothesis that there were no differences in outcome of implant treatment after sandwich osteotomy in the atrophic posterior mandible with interpositional autogenous bone block graft, compared with bone substitute material. A MEDLINE/PubMed, Embase and Cochrane Library search in combination with hand-search of selected journals was conducted. Two short-term randomised controlled trials with a split-mouth study design and low risk of bias...
A 48-year-old blacksmith presented to A&E after sustaining facial injuries while operating a metal punch press. Radiographs revealed the presence of a metallic foreign body within the cheek, but this was clinically impalpable. By using a neodymium rare earth magnet extraorally, it was possible to identify the position of the fragment several centimetres lateral to the entry wound. Triangulation of the fragment's position allowed retrieval under local anaesthetic through the original penetration...
COVID-19 has accelerated a reliance on virtual technology for the delivery of postgraduate surgical education. We sought to develop a regional teaching programme with robust quality assurance. Webinars were delivered on a weekly basis by subspecialty experts using Zoom™ augmented with interactive polling software. Trainee feedback comprised Likert item rating on content and delivery, free text comments and self-assessed confidence levels using visual analogue scale (VAS) scores. A focus group was...
The aim of this study was to evaluate retrospectively the accuracy of a protocol for completely in-house, computer-assisted, orthognathic surgery, generating resin printed intermediate surgical guides. A retrospective, observational study was made on a cohort of 15 patients treated consecutively from September 2017 to May 2019, who underwent bimaxillary orthognathic surgery planned with the same 3-dimensional program and whose surgical intermediate splints were obtained with the same all-in-house...
When patients attend the emergency department with facial fractures that require surgery and are immediately admitted, surgery can be delayed as theatre time is prioritised for other more urgent patients. One solution is to send the patient home and admit them as an elective patient at a later date. The aim of this study was to investigate the outcomes of patients admitted directly and those seen as elective patients following fracture of the mandible or zygomatic complex. Data were taken from the...

Abstract Objectives Increasing evidence suggests that aberrant expression of microRNAs (miRNAs) is involved in the pathogenesis of primary Sjögren’s syndrome (pSS). The aim was thus to characterize the miRNA profile in saliva, salivary gland tissue, and plasma from patients with pSS and compare findings with those of patients having Sjögren‐like disease (non‐pSS). In addition, to correlate miRNA levels and clinicopathological features of pSS. Methods miRNA real‐time quantitative polymerase...
Abstract Background This observational case‐control study was designed to investigate the frequency of oral lesions in psoriatic patients and to identify an association between mucosal involvement, the severity of the disease and a form of treatment. Methods One hundred twenty‐seven patients diagnosed with psoriasis were enrolled in this study from November 2018 to September 2019. The oral mucosa evaluation was based on the clinical appearance, location and morphology of the lesions. All...
