An uncommon case of inferior vena cava injury during atrial fibrillation ablation
AbstractA 64-year-old man underwent catheter ablation (CA) of atrial fibrillation with intracardiac echocardiography (ICE) assistance. As the probe was advanced toward the right atrium, sudden abdominal pain was felt by the patient with hypotension and tachycardia requiring fluids and vasopressors for hemodynamic stabilization. The inferior vena cava (IVC) was injured by the passing probe and open repair was then performed. To our knowledge, this is the first reported case of symptomatic IVC laceration...

Laparoscopic assisted submucosal excision of an intussuscepting colonic lipoma
AbstractColonic lipomas are the most widespread non-epithelial tumours of the large bowel. They often present with absent, non-specific or intermittent symptoms. Intussusception is the most common complication that may occur as a result of such a lesion. We present the case of a young female patient with a history of intermittent abdominal pain associated with nausea. She presented to the emergency department with subacute large bowel obstruction. A diagnosis of colo-colic intussusception due to...

A challenging case of a chyle leak following axillary lymph node clearance
AbstractChyle leak following axillary lymph node clearance is a rarely reported complication. We present a case of chylous leakage following axillary lymph node clearance, which was diagnosed on clinical grounds. Surgical re-exploration was undertaken due to ongoing high output. However, the chylous leak recurred post-operatively, if at a lower rate. The patient was subsequently managed successfully with conservative measures, primarily utilizing regular aspiration and compression bandaging to the...

A huge extrarenal cell carcinoma developing from a heterotopic renal anlage with distant metastases: case report and literature review
AbstractRenal cell carcinoma (RCC) is the most common type of renal tumor arising from the proximal renal tubules. Extrarenal RCC is a rare case in which a tumor is found outside the kidney with no primary kidney tumor. Some theories suggest that these tumors arise from mesoderm remnant. Here, we present a unique case, the fourth in literature, of peri-renal, Extrarenal RCC case and the first case with a huge mass with distant metastases and aggressive progression.

Recurrent lymphoepithelial cysts after parotidectomy in an undiagnosed HIV-positive patient
AbstractA benign lymphoepithelial cyst (BLEC) is defined as a rare, benign lesion composed of single or multiple cysts found within salivary glands or the lateral cervical area. Increasing reports suggest an association between BLEC and human immunodeficiency virus (HIV), although its pathogenesis remains unclear. We report a 51-year-old male who presented with recurrent bilateral BLECs after initial parotidectomy of cyst. An HIV panel was then performed with a positive result. We review the surgical...

Frontal sinus inverted papilloma managed with Draf III
AbstractInverted papilloma (IP) is a rare benign tumor that originates from nasal and paranasal sinus mucosa. Although it is considered a benign lesion, it is locally aggressive and has risk between 7 and 15% for malignant transformation. The management of frontal sinus (FS) IP is still challenging due to its rarity and local invasiveness. We report a rare case of IP originating from FS, and to review the literature to delineate the possible surgical strategy. A 52-year-old male presented with long-standing...

Metastatic epidural spinal cord compression
AbstractUp-to-date management for metastatic epidural spinal cord compression is based on appropriate scoring systems. In this case, separation surgery and stereotactic radiosurgery achieved an optimized outcome. A 75-year-old man had thoracic band-like pain, left lower limb weakness and difficulty ambulating for 6 weeks. Spinal images showed a T4 metastatic epidural spinal cord compression with a pathological fracture of T4 and T10 and L3 lytic lesions. There was no other neoplastic lesion. The...

De novo adolescent gastric carcinoma: a first case report in Saskatchewan, Canada
AbstractAdolescent gastric cancers are extremely rare with a reported incidence of 0.05–0.10% in North America. We present a de novo case of gastric carcinoma in a 17-year-old teenager with no concomitant family history or risk factors. His main clinical presentation included anaemia and melaena stools. Despite an extensive clinical workup that included a diagnostic laparoscopy, the tumour was deemed surgically irresectable, and he was started on a palliative chemotherapy protocol at the local paediatric...

Correlation of the changes in physical activity and clinical results following lymphatic microsurgery
Abstract Background The physical activity (PA) of patients may change after microsurgical treatment for lower extremity lymphedema (LEL). We investigated whether PA changes perioperatively and whether it influences the treatment result. Patients and Methods Sixty patients with unilateral LEL (56 secondary and 4 primary) underwent lymphatic microsurgeries. Patients were divided into two groups based on improvement in International Physical Activity Questionnaire Short Form (IPAQ‐SF) categories;...

Usefulness of a template‐based anterolateral thigh flap for reconstruction of head and neck defects
Abstract Background One of the challenges in head and neck reconstruction is to have an adequate understanding of the three‐dimensionalities of the defects created after resections due to the high variability of clinical scenarios. Consequently, it is essential to design the flap to match the requirements of the defect in order to facilitate the insetting and to achieve a successful outcome. The anterolateral thigh flap (ALT) is a robust and versatile flap commonly used in head and neck reconstruction....

Microsurgical reconstruction of extensive lower extremity defects with the conjoined parascapular and latissimus dorsi free flap
Abstract Background Extensive lower extremity soft‐tissue defects pose a reconstructive challenge. We present our experience with the conjoined parascapular and latissimus dorsi (CPLD) free flap. Methods From October 2008 to October 2017, 69 patients (14 female, 55 male) with a mean age of 50 years (range: 16–79 years) underwent reconstruction of lower extremity defects with the CPLD free flap. Mean defect size was 24 × 36 cm (range: 14 × 20 to 45 × 80 cm). Results Mean latissimus...
