In their multicentre, cluster-randomised controlled trial, Stine Bernitz and colleagues1 found no significant difference in the frequency of intrapartum caesarean section (ICS) between the obstetric units adhering to the WHO partograph and those adhering to Zhang's guideline. However, some inevitable biases would have existed since the assignment could not be blinded to health-care providers.
