Five‐year results of a modified left atrial maze IV procedure in the treatment of atrial fibrillation: a randomized study
In this study, we aimed to investigate the outcomes of a modified left atrial maze IV procedure (MLAM‐IV) for the surgical treatment of AF. Results showed that the MLAM‐IV is technically simpler than the traditional left atrial maze‐IV (LAM‐IV) in the ablation process. The MLAM‐IV was associated with less ventilation support in early postoperative period. The long‐term efficacy of the MLAM‐IV in the treatment of AF is similar to that of the LAM‐IV. Abstract Background The left atrial maze...

Incidence and predictors of common bile duct stones in patients with acute cholecystitis: a systematic literature review and meta‐analysis
Forest plot of from 19 studies on the prevalence of common bile duct stones in acute cholecystitis showing a pooled incidence of 13.7%. Abstract Background Acute cholecystitis (AC) is a surgical condition that is usually managed by emergency surgery. The presence of common bile duct stones (CBDS) in this setting mandates definitive treatment to avoid complications such as cholangitis. The incidence of CBDS in the setting of AC is poorly defined. Methods A systematic English literature...

Suprapubic penetrating abdominal trauma - defining peritoneal breach and choices in operative intervention
AbstractPenetrating abdominal trauma is an uncommon cause of presentation to emergency departments in Australia and is frequently associated with the clinical need for emergent operative intervention. Advances in imaging modalities, improved laparoscopic techniques and structured approaches to resuscitation in trauma have now allowed potential minimally invasive management of such injuries, avoiding laparotomy and therefore defining peritoneal breach; the major determinant of intra-abdominal organ...

Merkel cell carcinoma: a case report and literature review
AbstractMerkel cell carcinoma (MCC) is an aggressive cutaneous malignancy of neuroendocrine origin presenting as a painless, rapidly growing nodule. MCC often presents in elderly, fair-skinned individuals in sun-exposed areas. Diagnosis is often overlooked at time of presentation due to its rarity, but MCC is twice as deadly as malignant melanoma. There has been bigger interest in the disease due to increasing incidence and an association with the prevalent virus Merkel cell polyomavirus. This study...

A rare case of multi-focal non-contiguous necrotizing soft tissue infections and literature review
AbstractNecrotising soft tissue infection (NSTI) is a rare yet life-threatening surgical emergency, characterized by rapidly spreading infection below the epidermis to the soft tissue layers of the dermis, subcutaneous layers, fascia and sometimes muscle. Multi-focal NSTIs have been rarely described in the literature, with reported cases predominantly involving immunocompromised patients. We report a case of an immunocompromised 70-year-old woman who initially presented with concern of a perforated...

Endovascular treatment of iatrogenic iliocaval fistula post lumbar disc surgery
AbstractA 39-year-old patient presented with a history of right-side heart failure (dyspnea and orthopnea), right lower extremity deep vein thrombosis, and hepatic impairment. A physical exam showed increased jugular venous pressure, hepatomegaly, and bilateral lower limb pitting edema. A computed tomography angiograph revealed an arteriovenous communication between the right iliac artery and inferior vena cava (IVC) at the level of L4/L5. The patient was diagnosed with a large arteriovenous fistula...

Isolated pneumomediastinum following laparoscopic cholecystectomy: a rare complication
AbstractDevelopment of postoperative pneumomediastinum is one of the most infrequent complications of laparoscopic procedures. We report a case of a 47-year-old woman who developed pneumomediastinum consequently to laparoscopic cholecystectomy. The patient was treated conservatively and was discharged on the fifth postoperative day. Early detection of this condition, differential diagnosis and careful monitoring of the patient are important, as it may lead to severe consequences, including the life-threatening...

IMFT of the sigmoid colon: a case report
AbstractInflammatory myofibroblastic tumor (IMFT) of the colon is a very rare entity, characterized by proliferation of myofibroblast cells admixed with inflammatory infiltrates. The entity was first described in the late 1990s, and since then less than 30 cases have been described. It frequently mimics other neoplasm of the colon, such as adenocarcinoma, lymphoma and stromal tumor, and is very difficult to clinch the diagnosis preoperatively based on the clinical and radiological findings. Here,...

Use of endoloop in video-assisted thoracoscopic enucleation of a very rare esophageal tumor
AbstractA gastrointestinal stromal tumor is an infrequent tumor of the gastrointestinal tract with very rare involvement of the esophagus. We present a case of a patient with dysphagia and a 4 cm submucosal mass. The patient underwent thoracoscopic enucleation with complete resection of the mass. We present case details and operative video highlighting the important surgical steps of exposure and retraction. We believe that the Endoloop is a very useful tool in providing countertraction needed during...

Ischemic mitral regurgitation in a patient with dextrocardia and situs inversus totalis
AbstractDextrocardia with situs inversus is a rare condition. Few previously published cases have addressed the observation of isolated coronary artery disease or isolated mitral valve disease in these patients. However, the occurrence of ischemic mitral regurgitation in such cases has never been previously reported. Here, we report such a case with emphasis on both surgical and echocardiographic features.

Vaginal evisceration of small bowel
AbstractEvisceration of bowel through the vaginal vault is an extremely rare condition and is considered to be a surgical emergency. We present the case of an 83-year-old female who was brought to the emergency department with a vaginal prolapse complicated by evisceration of small bowel. A midline laparotomy was performed for reduction of the bowel with a subsequent vaginal repair. We present this case due to its rarity and high-reported mortality rate.

Superior mesenteric venous thrombosis as a first manifestation of Antithrombin III deficiency in the postoperative course of laparoscopic sleeve gastrectomy: a case study of 2 patients from 1211 bariatric patients
AbstractSuperior mesenteric venous thrombosis (SMVT) following laparoscopic sleeve gastrectomy (LSG) is a rare, potentially life-threatening complication, which presents either isolated, or as a part of portal/mesenteric/splenic vein thrombosis. Distinction between them possibly confers an important clinical and prognostic value. Antithrombin III (ATIII) deficiency causes an hypercoagulable state which predisposes to SMVT. We report the clinical presentation and treatment of two patients among 1211...

Neovaginoplasty for radiation-induced vaginal stenosis using Nile Tilapia Fish Skin as a biological graft
AbstractTilapia skin showed good results when used as a biological graft for surgical management of Mayer–Rokitansky–Küster–Hauser syndrome. Thus, our researchers considered the use of this biomaterial for neovaginoplasty in radiation-induced vaginal stenosis. We report the case of a 41-year-old female patient with a total occlusion of the vaginal canal after radiotherapy for vaginal cancer. McIndoe neovaginoplasty using tilapia skin as a scaffold for proliferation of new vaginal epithelium was performed....

Primary actinomycosis of the big toe: a case report and literature review
AbstractActinomyces are Gram-positive branching bacteria that are part of the human gastrointestinal microflora. These organisms can cause actinomycosis, which is a slowly progressive suppurative infection involving the cervicofacial region. Primary involvement of extremities is very rare. Here, we present primary actinomycosis of the lower extremity and a review of related literature. A 35-year-old female from Al-Kharj City (agricultural area) presented to our plastic surgery clinic with complaints...

Tongue necrosis: a rare complication of prolonged oral intubation
AbstractThis is a case report of necrosis of more than two-third of the dorsal tongue in a 74-year-old male following prolonged oral intubation for vascular surgery. This necrosis progressed from the left tongue to involve much of the dorsal tongue bilaterally. A diagnosis of rhabdomyolysis was made evident by compartment syndrome of the legs with an elevated creatine kinase level of 89 789 u/l (units per litre). The literature also reveals that vasopressin has been linked with reported cases of...

A rare presentation of an ectopic kidney with pyelonephritis mimicking appendicitis
AbstractAcute appendicitis is one of the top differential diagnoses of right lower quadrant pain in the emergency department. There are many other conditions that may mimic appendicitis such as diverticulitis, colitis and gynecological conditions. We report a rare diagnosis of a patient who presents with characteristic clinical and laboratory features of appendicitis with severe sepsis, but later showed acute pyelonephritis of a malrotated right ectopic kidney on computer tomography. An ectopic kidney...
