Trismus, or restricted mouth opening, is a well-documented complication after dentoalveolar or maxillofacial surgery that is daunting not only for the patient but also for clinicians to treat. It is usually caused by inflammation and spasms of the masticatory muscle postoperatively, or unyielding scarring. The limited opening of the mouth may cause problems with chewing, speech, and oral hygiene, and may cause pain.1
The choice of treatment for diacapitular condylar fractures remains in dispute among oral and maxillofacial surgeons. A multivariate retrospective study was designed to compare the prognosis after conservative treatment and surgery, and to explore further indications for management. From 1 July 2013 to 30 June 2017, 169 patients with diacapitular condylar fractures were included. Relevant preoperative data were collected, and the prognoses assessed. Three ordinal logistic regression models were constructed...

ABSTRACT INTRODUCTION To investigate the multifactorial processes underlying cognitive aging based on the hypothesis that multiple causal pathways and mechanisms (amyloid, vascular, and resilience) influence longitudinal cognitive decline in each individual through worsening brain health. METHODS We identified 1230 elderly (aged ≥50 years) with average 4.9 years of clinical follow‐up and with amyloid‐PET, DTI, and structural MRI scans from the population‐based Mayo Clinic Study of Aging....
Abstract Objective To identify circuits active during neonatal hypoxic‐ischemic (HI) seizures and seizure propagation using electroencephalography (EEG), behavior, and whole brain neuronal activity mapping. Methods Mice were exposed to hypoxia‐ischemia on postnatal day (p) 10 using unilateral carotid ligation and global hypoxia. EEG and video were recorded for the duration of the experiment. Using immediately early gene (IEG) reporter mice, active cells expressing cfos were permanently...

Objective To evaluate for differences in time to decannulation and survival rates for pediatric tracheotomy patients based on ventilator status upon discharge. Study Design Retrospective longitudinal cohort study. Methods A single‐institution longitudinal study of pediatric tracheostomy patients was conducted. Patients were categorized based on mechanical ventilation status on discharge and principal reason for tracheostomy. Survival rates were determined using the Kaplan‐Meier method....
Objectives/Hypothesis Introduction and widespread use of cricotracheal resection and anastomosis (CTRA) as routine treatment for high‐grade benign laryngotracheal stenosis (LTS) led to the need for a new classification system that could accurately predict surgical outcomes by integrating crucial stenosis and patient‐related information. In 2015, the European Laryngological Society (ELS) proposed a new classification for benign LTS. We retrospectively tested it in adults treated at three referral...
Objectives Vocal cord dysfunction (VCD) has been used by clinicians, primarily pulmonologists, to describe a variety of conditions in which the regulation and coordination of vocal fold movements are part of the explanation of cough or difficulty breathing, mainly paradoxical vocal fold motion disorder (PVFM). Prior studies show an intersection of mental health issues, primarily anxiety, and PVFM. We began incorporating mental health screening tools using the Life Events Checklist‐5 (LEC‐5) and...
Drain placement in thyroidectomy is associated with longer hospital stay without preventing hematoma
Objective To analyze the effect of drain placement on postoperative hematoma formation and other associated outcomes post–thyroid surgery in a large national cohort. Methods This was a retrospective study that analyzed data from the 2016–2017 National Surgical Quality Improvement Program (NSQIP) public use files. Baseline characteristics and perioperative outcomes were compared between drain and no drain cohorts. Results A total of 11,626 patients were included; 3281 had a drain placed...
Objective To determine whether the presence of a concomitant upper respiratory tract infection (URI) impacts upon Botulinum toxin (BoNT) efficacy in spasmodic dysphonia (SD) patients. Study Design Case series and literature review. Methods All SD patients with a concurrent URI, presenting for BoNT therapy at a clinical research center from November 2016 to December 2017 were included. A total of 12 patients were identified. Patients were followed for at least two BoNT treatment cycles...
Objective To compare healthcare utilization in voice patients with versus without mental health (MH) diagnoses Study Design Retrospective study using electronic medical records from large regional healthcare system. Methods We examined data on sociodemographic characteristics, comorbidities, voice‐related diagnoses, and patterns of healthcare utilization (including medication use, tests and procedures, and outpatient visits). The study period spanned January 2005 through June 2017. ...

Airway endoscopy (rigid and flexible bronchoscopy) is an important procedure that allows for visualization of the trachea and bronchi as well as treatment of a variety of airway disorders for diagnostic and therapeutic interventions. Excellent communication between the anesthesiologist and the endoscopist is required to ensure that adequate oxygenation and ventilation is maintained via the shared airway. Various anesthetic and airway management techniques can be used for airway management in pediatric...

Abstract Fetal micrognathia can be detected early in pregnancy. Prognosis of micrognathia depends on the risk of respiratory distress at birth and on the long‐term risk of intellectual disability. The purpose of this study was to evaluate the long‐term prognosis of fetuses with prenatal diagnosis of micrognathia by estimating the prevalence and the severity of confirmed genetic diagnosis in our cohort. Our retrospective study included 41 fetuses with prenatal diagnosis of micrognathia referred to...

Aeroallergen immunotherapy is a safe and effective disease-modifying treatment associated with rare therapy-associated fatality. Significant practice variation surrounds universal or contextual prescription of self-injectable epinephrine (SIE) for patients receiving AIT.Objective: To characterize the cost-effectiveness of a universal vs. contextual SIE requirement for patients receiving AIT.
A 15-year-old male presented with a new onset pruritic, papulovesicular rash three weeks following a minimally invasive pectus excavatum repair using a metal bar. The rash initially appeared on his upper extremities and chest wall around his surgical incisions (Figure 1A). Later, new erythematous lesions occurred on the penile foreskin. Patient denied systemic symptoms or prior history of metal or medication allergies.
Although atopic dermatitis (AD) has been associated with increased risk of food allergy (FA), this association is not fully understood and the predictors of severe FA are not clear. In a review of 18 population-controlled studies, the rate of food sensitization was up to 6 times higher in patients with AD versus normal controls.1 A recent study identified a potential physiologic mechanism by demonstrating increased skin barrier dysfunction, measured by increased transepidermal water loss and reduced...
Idiopathic systemic capillary leak syndrome (SCLS) is a rare disorder characterized by recurrent episodes of hypotensive shock due to vascular hyperpermeability. Since the disorder was first described in 1960, fewer than 300 cases have been reported. SCLS is a diagnosis of exclusion and the differential diagnosis is broad. Once a diagnosis is made, clear guidelines for the management and prevention of episodes are lacking, often leading to delays in care.
